Prairie State Rehab is a Chicago-based medical practice owned and operated by Dr. Jason Freeman, a board certified Physiatrist. The goal of the practice is to help patients with disabilities live their highest quality of life by optimizing their mobility. Recognizing that one of the greatest challenges for patients with disabilities is arranging transportation to an outpatient office, we alleviate this inconvenience by providing home visits and telemedicine services. We partner with hospitals and vendors to help patients get the equipment they need to improve their mobility. For some patients, this means helping them get the correct wheelchair; for others, the perfect custom lower extremity prosthetic. In all cases, the idea is the same: Improved mobility leads to improved independence and a higher quality of life.
Patients often experience delays in receiving equipment they need due to the complex paperwork and approval process for mobility equipment that currently exists. Spending almost a decade prescribing wheelchairs, prosthetics, and orthotics, Dr. Freeman has knowledge and experience navigating the process to get patients the correct equipment quickly. Having a background in Physiatry also helps Dr. Freeman understand when patients would benefit from additional treatments such as spasticity management, wound care, pain management, and Physical and/or Occupational therapy.
Physiatry is rooted in helping people with disabilities and identifying ways to help those patients live their most fulfilling lives. Prairie State Rehab is dedicated to honoring this idea and will continue to grow and find ways to help the patients who can benefit from their services.
Telemedicine:
Telemedicine has allowed us to expand our reach and help those patients who are homebound, unable to travel, or who live outside the Chicagoland area. Like all of our evaluations, telemedicine has allowed us to expedite the process for helping patient receive wheelchair, home equipment, and any other DME that may help make their lives easier. Utilizing our services, a patient no longer has to travel to an office multiple times to receive new equipment and instead can get what they need in a single telemedicine visit.
We can see patients in the following settings: Skilled Nursing Facilities, Group Homes, and Patients Homes.
We are allowed to see patients who are currently receiving Medicare Part A benefits while on homecare.
